INSTRUCTIONS FOR APPLICANT

You will be working with a Public Notary or Registrar/Trusted Correspondent from your organization or IdenTrust to get your Department of Defense (DoD) ECA certificate. As a part of the certificate application process, you will be asked to complete and sign an In-Person Identification form in the presence of the Public Notary or Registrar/Trusted Correspondent.

Step 1: Review The ECA Subscriber Agreement and Policies
By signing the In-Person Identification Form you are agreeing to the terms of the ECA Certificate Subscriber Agreement. You may review this agreement at the following URL: IdenTrust Services ECA Certificate Subscriber Agreement

In addition to the Subscriber Agreement, you are agreeing to the terms in the public version of IdenTrustís ECA Certification Practices Statement, version 1.0 dated November 8, 2006 ("CPS") Certificate Practice Statement (CPS) and the ECA Certificate Policy, version 3.1 dated October 12, 2006 ("CP").

Step 2: Ensure That You Have Appropriate Identification
You will need to present certain identification to the Registrar/Trusted Correspondent. U.S. Citizens must present:

One from List A and one from List B or C OR One from List B and one from List C (Non-US Citizens must present a valid passport and one form of identification from List B.)

LIST A - Photo ID Documents that Establish Identity and Citizenship LIST B - Photo ID Documents that Establish Identity LIST C - Other Documents that Establish U.S. Citizenship but not Identity
1. Passport from Country of Citizenship 1. Driver's license or governmentissued ID card (containing a photograph) 1. Original or certified copy of a birth certificate issued by a state, county, municipal authority, or outlying possession of the United States bearing an official seal
2. Certificate of U.S. Citizenship issued by U.S. Citizenship and Immigration Service, formerly the Immigration and Naturalization Service-USCIS (INS) 2. Military ID (with photograph) 2. Consular Report of Birth from a U.S. Consulate (Form FS-240)
3. Certificate of Naturalization issued by a court of competent jurisdiction prior to October 1, 1991, or the USCIS (INS), since that date 3. Permanent or Unexpired Temporary Resident Card issued by the USCIS (with photograph) 3. Certification of Birth Abroad issued by the Department of State (Form DS-1350)


Step 3: Complete Forms with a Public Notary or Registrar/Trusted Correspondent
Meet with the Public Notary or Registrar/Trusted Correspondent and work with him or her to complete the required forms.

ECA ID Form: In-Person Identification Form (Registrar/Trusted Correspondent)

Step 4: Submit Your Forms
Give the original In-Person Identification form to your Registrar/Trusted Correspondent and keep a copy for your records.
